CLINICAL
Galaxy maintains virtually all historical data for the Electronic Patient Record. This includes a history of diagnoses, including acute episodes; complete medication and physician order history; complete assessment history, and the tracking of changes to those assessments over time. You can even retrieve a Care Plan as it existed at any point during a resident's stay. Galaxy employs expert data mining to utilize this integration to provide an incredibly accurate, efficient and cross-checked clinical picture, resulting in the most practical and effective path to higher reimbursement, better resident care, and a genuine reduction of the work load.
MDS
Galaxy's MDS is as easy as it gets. The data entry screens look just like
the paper forms. The system knows in advance the due dates for completion of
each resident's next Assessment, based on Regulations and Facility Policy,
as well as the range of allowable Assessment Reference Dates (ARD). Galaxy's
MDS allows for gathering additional detail which helps determine which
legitimate answers will enhance RUGs scores, aid in the completion of CAAs,
and flow directly to the Care Plan. In addition to preventing submission
fatal errors or warnings, other inconsistencies, even between different
sections, are flagged for your consideration. Galaxy "does" the CAAs for
you, virtually instantaneously. Galaxy's Case Mix Advisor™ analyses the ADL
score and RUGs calculation to promote the highest reimbursement.
The Galaxy MDS is Bi-Lingual. MDS can be entered and printed in both English
and Spanish, an important consideration in some areas for resident and
family involvement addressed in the QIS Survey.
Also included with Galaxy's MDS is MaxTrax™.. For residents whose next
Assessment will be used to determine the RUGs score, MaxTrax™ tells you what
MDS items need to be tracked for which residents for days which may fall in
the Assessment Lookback Period. It prompts your STNAs to enter ADL and other
MDS data during each shift into hallway-located touchscreens, handheld
devices, or tablets, and then determines the best ARD. The result is a set
of well-documented MDS answers for the highest RUGs scores.
Galaxy's MDS is fully integrated with the Galaxy Care Plan module. You can
jump from any section in the MDS (including CAAs) to Care Plan. With one
click, you can view a complete draft Care Plan, exactingly individualized
for that resident.
MDS reports include the 672 (Census and Conditions), 802
(Roster/Matrix), Weight Variance, Due Dates, Medicare/Medicaid, and user
definable statistics.
CARE PLAN
Galaxy’s Care Plan is fully integrated with the Galaxy MDS. Not only can the MDS create a draft Care Plan, but on-going changes to the Care Plan can inform the MDS of potential answer changes, and even signal a possible Significant Change, which can have reimbursement implications.
The Care Plan can be initiated and updated by the MDS, or started from scratch and updated and edited directly. You enter the Care Plan on a screen that looks like a Care Plan, with separate columns for Problems, Goals, Interventions, Disciplines, etc. While Galaxy allows the user free typing at any stage of data entry, most, if not all, of the Care Plan is created by simple clicks. Because Galaxy breaks down the Problem into multiple components (Problem Statement, Extension, Related to, and Resulting Symptoms), creating a resident-specific Care Plan is possible with a few clicks and virtually no tedious typing. Galaxy’s clever logic allows the Care Plan to avoid the non-natural use of repeating the resident’s name countless times, with judicial use of he, him, his (or she, her – as the case may be), and not the impersonal “he/she”.
In addition to easy templates for resident preferences, Galaxy’s Care Plan identifies Risk Factors for each Problem addressed - as well as Strengths present that are necessary for selected Interventions - automatically, without requiring additional user entry, and yet it is very specific to that resident.
Galaxy provides an already set up database of Problems, Goals, etc., designed by Ms. Debbie Ohl, nationally known consulting nurse (and LNHA), and author of The Big Book of Care Plans, published by HC Pro. The Language Master has been designed to integrate into MDS 3.0 and to exceed requirements of the QIS Survey.
The Galaxy Care Plan is Bi-Lingual. Care Plans can be created and printed in both English and Spanish, an important consideration in some areas for resident and family involvement addressed in the QIS Survey. The Care Plan Module includes automated translation assistance when entering new text and Master Language.
The Care Plan System integrates with Billing as well, and can be used to cross check charges actually billed against charges anticipated by the Interventions in the Care Plan.
The Galaxy Care Plan module provides a host of reports, including Assignment Sheets and Statistical reports.
PHYSICIAN’S ORDERS AND ELECTRONIC MEDICAL ADMINISTRATION RECORD
Galaxy maintains a drug database, updated monthly, that allows entry of
medication orders with several levels of automated drug-to-drug interaction
and off-label warnings. Appropriate frequencies and routes of administration
accompany your selection of a medication, and stop days can be predetermined
according to state regulations and facility policy. Printed MARs include a
place to enter vitals (pulse/BP, etc.) that may be required for a particular
drug, special formatting for PRN meds, and a narcotic count sheet where
appropriate. User customized Treatment, Dietary, and Lab order capability
eliminates re-typing commonly used orders for multiple residents. You can
also choose to have Galaxy automatically print Black-box Warnings and
side-effects on the reports as well. Galaxy provides statistical reports
valuable for Quality Assurance, research, and organizational use.
The Galaxy Medication Leaflet is Bi-Lingual. Leaflets containing important information about each prescription can be printed in both English and Spanish and shared with the resident and family.
Telephone orders are printed on a special perforated sheet with peel-off labels to update mid-month printed Physicians Orders, MARs and TARs, eliminating much handwriting. One perforated part fits neatly into a double window envelope to send to the Physician for signature. The System will pop up a daily alarm for order signatures still due.
When you have a single med cart delivering multiple medications to 40 or more residents for, say, 8:00 AM, it is difficult, if not impossible, to deliver all meds to all residents within the legal time limit. Galaxy offers Automatic Time StaggeringTM, which calculates the realistic time to serve each resident. Based on a pre-determined order of residents (not necessarily simply by room number), the System will automatically increase the time the Physician’s Order calls for the resident to receive the meds in 15 minute increments, as the med cart moves along its route.
ASSESSMENTS
Galaxy’s Visual Assessment Builder allows you to create unlimited custom Assessments. You have complete cosmetic control of the Assessment layout, including number of columns, organization of related questions in special titled boxes, font styles and sizes, even the use of graphic images.
You can incorporate various types of data fields: check box, fill-in-the-blank, multiple choice, etc. Each Assessment can have any number of user-definable questions. For each check box and multiple choice question, you can establish a scoring method.
In addition to the Assessment itself, the System prints a variety of reports. You can view and compare historical Assessments for a given resident, Assessment, and date range. You can produce statistical reports based on virtually every objective question, and view Quality Assurance statistics based on historical Assessments.
